You can walk into our clinics or schedule an appointment by calling 225-925-1906.
What to Bring to Your Appointment
On your first visit, please bring the following:
– Your social security card
– Your insurance card
– A photo ID (driver’s license or official government ID)

For your convenience and to save time, please print and fill out the forms prior to your appointment. These forms will also be available to you in the clinic if you prefer to fill them out when you arrive.
